When a tooth is broken below the gum line or has deep decay extending under the gum, partial orthodontics (MTM) can gradually pull the tooth into a position where it can be treated or restored.

This method can be a last option to save teeth that might otherwise need to be extracted.

Treatment Process & Duration

    Using a dental microscope, we can observe fine structures and lesions that are difficult to see with the naked eye.
    This allows for more accurate examination, diagnosis, and treatment.

  • ②Placement of Orthodontic Device

    Brackets and wires are used to apply gentle force to the tooth.

  • ③Extrusion

    The tooth is gradually pulled upward over several weeks.

  • ④Stabilization and Shaping

    The tooth is stabilized in its new position and shaped as needed.

  • ⑤Final Restoration

    The tooth is restored with a crown or other prosthetic restoration.

The overall treatment period typically ranges from approximately 2 to 4 months, depending on the case.

Extrusion Treatment Case

A case where a front tooth was fractured and there was insufficient ferrule for restoration.

Using a dental microscope, the fracture line was carefully located, and the tooth was gradually extruded and stabilized over 2–6 months while monitoring mobility.
As shown in the diagram, a wire was embedded in the root and connected to the temporary crown with a wire and elastic to gently pull the tooth upward.

After extrusion, the gum shape can be adjusted as needed, allowing for final crown restoration.
